How to Log In to GTM Forms

Overview

This article explains how to log in to GTM Forms and access internal documents and statements, including the 4138 Document and IVA (Initial Veteran Assessment) Document.

Step 1: Open the GTM Forms Login Page

Open your web browser and go to:


You will be taken to the GTM Forms sign-in screen.

Tip: If you are already on the GTM Forms homepage, click LOGIN in the top-right corner of the page to access the sign-in screen.

Step 2: Enter Your Login Credentials

On the login page, enter the following:


  • Email
    Enter the email address associated with your GTM Forms account.

  • Password
    Enter your account password.

Optional

  • Remember me
    Select this option if you are using a private or trusted device and want to remain signed in.

Step 3: Click “LOGIN”

Click the LOGIN button to sign in.

If your credentials are correct, you will be redirected to the GTM Forms dashboard.

Step 4: Access Internal Documents

After logging in, you will see the GTM Forms dashboard.

From the Our Internal Documents section, you can:

  • Open the 4138 Document
  • Open the IVA (Initial Veteran Assessment) Document
  • View other available internal forms and resources

To open a document, click the document icon on the corresponding card.

Step 5: View and Manage Statements

Once logged in, you may also access the Statement List, where you can:

  • View existing statements
  • Check statement status (Pending, Missing Information, Completed)
  • Select Actions to view, complete, transcribe, or manage statements (based on your permissions)

Forgot Your Password?

If you cannot remember your password:

  1. Select Forgot password? Reset on the login page
  2. Enter your email address
  3. Follow the instructions sent to your email to reset your password

Troubleshooting Login Issues

If you are unable to log in, try the following:

  • Confirm you are using the correct email address
  • Ensure your password is entered correctly (passwords are case-sensitive)
  • Reset your password if needed
  • If issues persist, contact Support for assistance
